Application Process

Step 1: First, read the Statutes, Rules, Regulations, and Policies linked here (https://www.mass.gov/lists/statutes-and-regulations-podiatry). Your signature on the application will attest that you have completed this requirement.

Step 2: Request your Official Final Undergraduate Transcript and Official Final Transcript from Podiatry College to be sent directly to PCS from your educational program.

Step 3: Contact all states where you have been licensed to request Verification of Licensure, indicating that you are in good standing. This is necessary whether the license is current or expired. Ensure documentation is sent to PCS.

  • Electronic Transcripts and/or Verification Letters: PCS will accept final official electronic transcripts and verification letters received directly from the primary source (such as the School, Parchment, the National Student Clearinghouse, and the State Licensing Board) via secure server to mapodiatry@pcshq.com.
  • Transcripts and/or Verification Letters via Postal Mail: Official Final Transcripts and/or Verification Letters may be mailed by the educational program or State Licensing Board directly to PCS at:

Step 4: Request a Certified Official Transcript indicating Passing Scores in NBPME Parts I, II, (Limited) III (Initial and Reciprocity) to be sent to PCS.

Step 5: Go to the Mass.gov website to complete the online application and submit fees. Before starting your application, have the following information available:

  • A standard 2×2 photograph portrait which must be passport style with a solid background. Driver’s license photos are not accepted.
  • Payment will be made by credit card. All fees are non-refundable and non-transferable.

Step 6: Request three letters of reference from individuals you have known for at least three years. None of these references can be relatives. Letters must be submitted by the reference either electronically or by mail.

Step 7: Download and submit the Residency Program Affidavit. CLICK HERE

Step 8: Verify your Masshealth Enrollment at https://www.mass.gov/information-for-masshealth-applicants.

Step 9: Complete the CORI Acknowledgement Form in the presence of a Notary public prior to application completion.

Step 10: PCS will review your application upon receipt of all required documentation and notify you via email of any discrepancies.

Step 11: Once approved, you will receive Authorization to Test (ATT) via email from PCS.

Step 12: Schedule and take the Jurisprudence exam within 90 days of the ATT’s issuance. Failure to take the examination within 90 days will result in a voided ATT.

Step 13: If you pass the exam, you will receive a License Letter via email from PCS once the CORI has been completed in 1-3 weeks. If you do not pass, you will receive a score notice with re-examination instructions via email.

Step 14: Once the CORI has been approved and the License fee has been paid (if applicable), you will receive a licensure confirmation email and can confirm your license number on the Massachusetts Office of Health and Human Services License Verification Site at https://checkahealthlicense.mass.gov/.


Fees

  • Limited Licensure — $270
  • Initial Licensure — $385.00
  • Issuance of Initial Licensure — $86.00 (payment made after successful completion of jurisprudence exam and National Boards, Part III exam).
  • Reciprocity fee — $385.00 (send with the application)
  • Issuance of Licensure by Reciprocity — $86.00 (payment made after successful completion of jurisprudence exam).

PART III EXAMINATION: If the applicant has not yet taken and/or been successful at the Part III examination they must do so. Applicants must schedule with and submit their $900.00 examination payment directly to Prometric. Please visit https://myaccount.prometric.com/login/nbpme/apmle to create an online account or log into an existing account (applicants that have previously created one).
