Q. There was an error on my application. How do I resubmit my application without payment?

Q. I am having difficulty opening some of the forms. What can I do?

Q. What if I answered yes to a Good Moral Character (GMC) question?

A. Please CLICK HERE for further information.

Q. What if I have a change of address and/or name change?

Information Change Request Form - complete form and submit to the address posted at the top of the form.

For a name change, submit your request in writing with an enclosed copy of the legal name change document (i.e. marriage certificate, etc.). Send this to PCS (see contact information).

Q. Can I still apply for a reciprocal license if I don't currently hold a license?

A. Yes. Applicant must have graduated from a Board approved nursing program, have taken and passed the National Council Licensure Exam, and have been licensed by a Board of Nursing in the US.

Q. What do I do with the Verification of Nurse Licensure by Reciprocity page of my application?

A. Complete this form only if your original state of licensure or any other state you hold a license for is not a NURSYS participating state.

Q. What do I need to do if a State Board will not verify my temporary license?

A. If a State Board will not verify a temporary license, we need a letter from that State Board stating that they do not verify temporary licenses.
