Q. What is the deadline for submitting my Scheduling Form to PCS?
A. There is no longer a deadline for submitting your completed Scheduling Form to PCS. Simply submit your Scheduling Form at least 30 days prior to the date you wish to test.

Q. How will I know that I am scheduled for the examination?
A. Once your Scheduling Form has been processed by PCS, a scheduling file will be transmitted to PSI. PCS will then send an authorization to test letter to you. In this letter, you will be instructed to contact PSI to set your testing appointment.

Q. How does an applicant from another state become licensed in PA?
A. Contact the Board for specific instructions. The telephone number is 717-783-3397.

Q. Where do I mail my Case History Record and Preceptor's Affidavit?
A. Mail your completed Case History Record and Preceptor's Affidavit to: PCS Attn: PA Funeral Directors PO Box 198689 Nashville, TN 37219-8689.

Q. When should I mail my Case History Record and Preceptor's Affidavit?
A. Mail your completed Case History Record and Preceptor's Affidavit to PCS on or after your Board approved Internship expiration date. Do not have your Preceptor's Affidavit notarized before this date.

Q. Can I send my materials to you overnight?
A. Yes. The overnight mailing address is: PCS Attn: PA Funeral Directors 150 Fourth Avenue North 8th Floor Nashville, TN 37219.

Q. What happens after I pass the examination and complete my internship requirements including submitting my Case History Record and Preceptor's Affidavit to PCS?
A. Once PCS has a complete candidate file (including passing score notice, Case History Record and Preceptor's Affidavit) the complete file is forwarded to the Board and the Licensure application is forwarded to the candidate by PCS. Candidates should complete this application in a timely manner and submit it to the Board office.
