Type 6 Funeral Director and Embalmer - Downgrade
Requirements:
A copy of current Registered Licensed Funeral Director license.
Complete the application including 2x2 photo.
An affidavit completed by the employer.
Criminal Offender Record Information (CORI) Form
Send payment of specified fees.

The completed application, application fee, and supporting documents are to be returned to PCS. The candidate will be sent a discrepancy letter if the application is incomplete. PCS will then forward the application and supporting documents to the MA Board. The MA Board will notify PCS of approved and disapproved candidates.
PCS will notify candidates of the Board's approval. The license numbers will then be posted on the Board's web site within five business days. The wallet license will be mailed by the Board within 6 weeks.

Payments may be made with a VISA, MasterCard, certified check or money order. Please make certified checks or money order payable to Professional Credential Services and include your SSN on the front of the payment. Fees are non-refundable and non-transferable.
