Own original stock certificates showing 10% ownership.
Submit copies of stock certificates for the Board files.
Submit the minutes from the stockholder meeting that shows transferal of ownership was approved.
Submit a letter from legal counsel for the funeral home showing the new distribution of stock ownership resulting from the transaction.
Mail one copy of your application to each Board Member.
Have the owner of the funeral home accompany the candidate to the Board meeting.
Submit copies of all license certificates currently held.

How To Obtain Application Materials:
You may call PCS at 1-877-887-9727 to request an application or you may download one here. (The application requires Adobe Acrobat Reader. Download Adobe Acrobat Reader for free. Click here to install it).
The completed application, application fee, and supporting documents are to be returned to PCS. The candidate will be sent a discrepancy letter if the application is incomplete. PCS will then forward the application and supporting documents to the MA State Board for approval. The Board will notify PCS of approved and disapproved candidates.
PCS will notify candidates of the board's approval. The license numbers will then be posted on the Board's website within five business days. The wallet license will be mailed by the Board within 6 weeks.
NOTE: Candidate must have a current Type 6, Type 5, or Type 1 and Type 4 Registration to apply for a Type 3 Registration.
