I just moved to Massachusetts and I want to transfer my current license (Reciprocity).  What do I need to do?
 
 
Requirements:

Complete the application including 2x2 photo.

Submit copies of your Birth Certificate, High School Diploma, Embalming/Funeral Director School Certificates and National Conference Examination Certificate.

Submit the name of funeral home in MA with which you intend to be affiliated.

Provide certification from your State Board stating the date you passed as an Embalmer and Funeral Director, and also the years you have served your apprenticeship.

Enclose a copy of your current license(s).

Submit a certification of good standing from your State Board.

Submit a resume of your job experience in the field.

Criminal Offender Record Information (CORI) Form

Send a certified check or money order made payable to PCS.

 

How To Obtain Application Materials:

You may call PCS at 1-877-887-9727 to request an application or you may download one here. (The application requires Adobe Acrobat Reader. Click here to download and install Adobe Acrobat Reader for free.)

The completed application, application fee, and supporting documents are to be returned to PCS. The candidate will be sent a discrepancy letter if the application is incomplete. PCS will then forward the application and supporting documents to the MA State Board for approval. The Board will notify PCS of approved candidates. The MA Board notifies unapproved candidates. All candidates are required to take and pass the Jurisprudence exam and the Practical exam.

If approved for the examination, candidates will receive an Approval letter along with a PCS scheduling form that must be completed and returned with the examination fee to PCS. PCS will send an Authorization letter with instructions. Candidates have until the expiration date indicated on the Authorization letter to take and pass both examinations at an authorized test center. Candidates that do not take and pass the examination within this period, forfeit all funds paid and are required to re-apply as a first-time candidate with PCS. Candidates that take the examination and fail within the allotted time are required to re-apply with PCS and pay the applicable re-exam fees.
Candidates with an Authorization Letter will call PSI to schedule the computer-based examination.
PCS will notify candidates of their pass/fail status. Passed candidates will be provided with instructions on the swearing-in ceremony before the Board. The Board will notify PCS of sworn-in candidates. Sworn-in candidates will contact PCS to pay for their license. The License numbers will be posted on the Board's website within 5 business days of receipt of license fee. The wallet license will be mailed by the Board within 6 weeks. Unsuccessful candidates will receive their score notice and rescheduling information (telephone registration).

Application fee : $339

Examination fee : $166 ($83.00 for each part)

License fee: $66

Payments may be made with a VISA, MasterCard, certified check or money order. Please make certified checks or money order payable to Professional Credential Services and include your SSN on the front of the payment. Fees are non-refundable and non-transferable.
